HPI:
@NAME@ is a @AGE@ @SEX@ with a history of intractable chronic migraine who presents to Neurology clinic for BOTOX injections for chronic migraine. @HE@ would like to proceed with injections as planned today. Please see detailed BOTOX procedure note below.
BOTOX PROCEDURE NOTE:
A concentration of BOTOX 100 units mixed in 1 ml sterile, preservative free saline was prepared for injection. Isopropyl alcohol was used for antisepsis. Prior to the injection patient was warned of potential adverse effects including transient pain and weakness. Patient ID confirmed.
Using a 30G needles the following muscles were injected:
Corrugator- 5 units each side (10 units total)
Procerus (mid)- 5 Units
Frontalis- 10 Units (divided in 2 locations) each side (20 units total)
Temporalis- 20 Units (divided in 4 locations) each side (40 units total)
Occipitalis- 15 Units (divided in 3 locations) each side (30 units total)
Cervical paraspinals- 10 Units (divided in 2 locations) each side (20 units total)
Trapezius- 15 Units (divided in 2 locations) each side (30 units total)
Total Units injected: 155 Units
Total Units wasted: 45 units
The patient tolerated with procedure well without complications and was discharged from the clinic
